1. Please download   Preparticipation Physical Evaluation (PPE) pages 1, 2, 3 and 4. 
2. Parent and athlete to both complete forms (Page 1, 2), including important medical information pertaining to your student athlete that the school will need to know. 
3. Bring all four (4) pages  to the licensed practioner for physical examination in timely manner prior to sports season.   
4. Student or parent shall submit  ONLY PAGE  4 to the school athletic office as part of  the athletic registration process.
