High School Online Registration

CHSAA Sports Physical/Preparticipation Physical Examination Forms

  1. Access forms through the QR code or download forms.
  2. Parent and athlete both complete forms (Page 1, 2), including important medical information pertaining to your student athlete that the school will need to know.
  3. Bring page 1-4 of forms to licensed practitioner for physical examination in timely manner prior to sports season.
  4. Turn in ONLY PAGE 4 to high school athletic office.

District Athletics is excited to offer this new opportunity for student-athletes and their families.  Now families may register their student for sport online and avoid printing off the forms and waiting in the long lines at the athletic office. 

Important!   Please create an account.   Choose your high school and scroll to the bottom of the page and you will be prompted to create an account.  After creating an account,  you will have access to the registration page.  

Please complete all the information before exiting the registration.  You cannot save a partially completed registration. Please have the following information available before you begin the process:

  • PSD Student ID#
  • Date of last physical exam for student athlete
  • Emergency contact information
  • Medicine, tetanus booster and allergy information

Payment can be accepted two ways:

1-  Online with a credit card using the SchoolPay (PSD online payment system).  http://psdschools.schoolpay.com/

**Please understand if a credit card is used for payment, you will be charged a small processing fee for the transaction.  There are no refunds under any circumstance for the processing fee.  Of course, you may choose to pay with cash or check to avoid the credit card processing/convenience fee.  

2- Cash/check can be accepted by the high school athletic office. 

Does your student need a sports physical or other medical or mental health services? 

The Health and Wellness Center at Centennial High School and virtual care program at Lincoln Middle School offers sports physicals/annual exams, sick visits, mental health services etc. year-round. The Center has a $0-$35 sliding scale (for qualifying families), accepts Medicaid/CHP+, most insurance plans and offers fee-for-service. No co-pays / deductibles are required.
