Preston ATHLETIC REGISTRATION & FEE INFORMATION

All middle school students participating in PSD Athletics must complete the PSD Middle School Sports Registration Process.  Follow the steps below to fully register your child for sports at Preston.   Keep in mind that the online registration process still requires families to submit a copy of the physical form, complete with a doctor's signature.  Please remember that students cannot participate in any athletic practices at Preston until the front office receives the medical release form signed by a doctor.   It is recommended that you begin to think about making appointments for your yearly medical/physical exam well in advance of the season, so you have plenty of time to see a doctor to obtain a signature before the sport begins.  In addition, athletic registration also includes a fee of $140 (per student, per sport) for Preston Students (See Unified Basketball fee below.  Payments can be made via SchoolPay or with a check payable to Preston Middle School.  

FOLLOW THE STEPS BELOW TO COMPLETE THE REGISTRATION PROCESS:

1. Register Online by clicking on the Blue Bar above the Preston logo.

  • For help creating an online account with Sports Engine, download the quick start guide.

2. Complete Required Annual Physical Exam Form

  • A physical examination by a doctor and a hard copy of the physician signature form needs to be submitted in the office prior to participation.  ATTACH A COPY OF YOUR PAID RECEIPT IF PAYING THROUGH SCHOOL PAY.
  • All physicals are valid for 365 days and expire one year from the original date of completion.  This step must be completed annually.

3. Payment of the $140 Participation Fee (per sport) Bring a copy of the School pay receipt when turning in your physical

  • Payments due prior to the first day of practice/tryouts

  • ​Payments can be made online via SchoolPay or with checks made payable to Preston Middle School 

  • For cut sports - refunds will be issued for students who do not make the final roster

  • If a student chooses to quit a sport, the following PSD refund policy will be followed:

Full Refund - For athletes who participate 5 days or less in a sport

1/2 Refund - For athletes who participate in more than 5 days of practice, before the first competition/game

No Refund - For athletes who participate in one or more competition/game.

 

Non-Preston Students: *Home school students or students who attend a school that does not offer middle school athletics will be placed upon request per the PSD Placement Guidelines.  FEE: $210.

1)Interested parents must contact the PSD Athletic office at 970-490-3332 to request formal athletic placement.  PSD will email acceptance to parent and Preston's athletic office.  

2) Once that is received, call 970-488-7303 to receive a "student id number" to register in Sports Engine. (We do not accept paper registration) At the end of registration, the fee can be paid online, or Fees and physicals can be dropped off at the front office at Preston. Physical Form

3) If you choose to email, call the attendance secretary to confirm.  You will receive a blue card once all of this is completed. Students must check into the front office to check-in, and can collect blue card on the first visit if registration is done electronically.

**** WAIVED FEES INFORMATION *****

  • Do you qualify for waived fees?  You can submit a request for waivers here.  Once you have qualified for waived fees you must log into Parent Vue, click on forms, and click permission to share under your student’s name. This allows us to waive the athletic fee.
    • PLEASE NOTE: If you are on waived fees you must still process a payment through SchoolPay (log into SchoolPay, select your students sport, add that sport to the cart and it will show up at $0.00 dollars due), process the payment and bring receipt in to the main office. 

SPORTS PHYSICALS

Does your student need a sports physical and you don't have a Primary Care Physician or are having trouble getting an appointment? The Health & Wellness Center (operated by Every Child Pediatrics) is OPEN and appointments are AVAILABLE for ANY PSD student for Sports Physicals.

  2. Centennial High School
