Poudre School District offers a comprehensive athletic program for students at the middle school level.  All of the middle school athletic programs are open to any student in 6th-8th grade (depending upon sport) who have completed the check-out process outlined below. 

Students in 6th grade are  allowed to participate in cross country, golf, tennis, wrestling and track/field at their respective middle school.  For all other sports at the 6th grade level, programming is offered through the City of Fort Collins Recreation.

Check-Out Process/Forms:

Families generally have two choices to register a student-athlete for a sports season.   (Please note-- some middle schools are asking families to only register online.) 

  1. Register online by navigating to the  Middle School Registration tab then to your designated middle school.
  2. Download and print all of the forms and submit them to the middle  school athletic office in person.  All middle school students participating in PSD Athletics must have the following forms completed and turned into their school prior to their participation.

Does your student need a sports physical or other medical or mental health services? 

The Health and Wellness Center at Centennial High School and virtual care program at Lincoln Middle School offers sports physicals/annual exams, sick visits, mental health services etc. year-round. The Center has a $0-$35 sliding scale (for qualifying families), accepts Medicaid/CHP+, most insurance plans and offers fee-for-service. No co-pays / deductibles are required. No student enrolled for care is ever turned away for an inability to pay for services provided at the Center.  For more information, visit the website at https://hwcenters.org/ or call the (970) 488-4950

Each student-athlete shall complete all forms in order to participate.

 

Middle School Athletic Fees 2023-2024

Please note there is no longer a family maximum.

  • PSD Students 6th-8th grade:
    • $140 per sport, $70 Unified Basketball
  • Home-Based, Charter, and Private School Students 6th-8th grade:
    • $210 per sport, $105  Unified Basketball
