Middle School Sports

Poudre School District offers a comprehensive athletic program for students at the middle school level.  All of the middle school athletic programs are open to any student in 7th-8th grade who have completed the check-out process outlined below. 

Check-Out Process/Forms:

To register your student-athlete for the upcoming sports season, please see the Middle School Registration tab at the top of the page.  

Families generally have two choices to register a student-athlete for a sports season.   (Please note-- some middle schools are asking families to only register online.) 

  1. Register online by navigating to the  Middle School Registration tab then to your designated middle school.
  2. Download and print all of the forms and submit them to the middle  school athletic office in person.  All middle school students participating in PSD Athletics must have the following forms completed and turned into their school prior to their participation.

SPORTS PHYSICALS Does your student need a sports physical and you don't have a Primary Care Physician or are having trouble getting an appointment?  

The Health and Wellness Center at Centennial High School will be offering sports physicals/well child exams over the summer months (June - August).  Any PSD middle and high school student (and their PSD enrolled siblings) can now enroll and be seen at the Center year round.   The Center prioritizes meeting the needs of students/families who are uninsured, underinsured and/or having a difficult time accessing medical care.   The Center accepts insurance (including Medicaid and CHP+), offers a cash pay option and a sliding fee scale (for qualified families).  No child is ever turned away for inability to pay.  Se habla español! Middle School Athletic Fees

Please note there is no longer a family maximum.

  • 6th, 7th and 8th grade:
    • $105 per sport
    • $63 for unified basketball (6th-8th grade only at Blevins, Boltz, Kinard, Lesher, Lincoln and Webber)
  • Home-Based, Charter, and Private School Students 6th-8th grade:
    • $157 per sport
    • $94 for unified basketball (6th-8th grade only at Blevins, Boltz, Kinard, Lesher, Lincoln and Webber)

Unified Basketball

Students in 6th grade are now allowed to participate in cross country, golf and track and field at their respective middle school.  For all other sports at the 6th grade level, programming is offered through the City of Fort Collins Recreation.
